Make the Sign of the Cross
Say “Forgive me Father (or Bless me Father) for I have sinned.  My last good confession was _____ ago
Then tell your sins to the priest
When you are finished telling your sins say:
“I am sorry for all of these sins and the sins I may have forgotten”
The priest will talk to you and will give you a penance.
    Then the priest will say
“Now make a good Act of Contrition”.
Then you say the Act of Contrition.
“O my God I am heartily sorry for having offended Thee.  I detest all my sins because I dread the loss of heaven and the pains of hell.  But most of all because I offended You my God who are all good and deserving of all my love.  I firmly resolve with the help of Your grace to confess my sins, do penance and to amend my life. Amen.”
After you say the Act of Contrition the Priest will give you absolution (forgives your sins).

     Then the priest says
Give Thanks to the Lord, for he is good”.

    The priest says,
The Lord has freed you from your sins. Go in Peace.”
You Say “THANKS BE TO GOD”.

Then you can leave and say your penance.
